TO REQUEST REIMBURSEMENT FOR PHYSICS 123

The following information needs to be submitted to the Physics Department Business Office:

1) Full Name

2) University ID number

3) Address to which the check should be mailed

4) Original receipt(s)

Once all information has been submitted to the Physics Business Office, a voucher will be processed and the student should receive a check in approximately two weeks.
